Person Centered Care Services is a not for profit organization creating social change within communities by supporting people with disabilities on their search for identity and acceptance.

Services Provided

Day Supports

Day Supports provides support for young adults who want to develop friendships and relationships with peers and people in the community, learn how to better read and write, and/or build volunteer experience and work-ethic in places where they would like to work or give back to the community.

Residential Supports

The people PCCS serves are able to live in a place they can call home, where they feel safe, independent, and supported by staff, family, and other loved ones. Our Residential Supports include two group living supports.

Independent Living Supports

Having a place to call home is a basic right that all people deserve. Yet people with disabilities often go a long time without finding a place to live. Discrimination, among other things, often prevents or delays the process of setting up living arrangements.

Career Supports

With education and support, people with disabilities fill intern, part-time, and full time positions.

Community Habilitation

PCCS Community Habilitation program provides all people with a wide range of services and resources that offer unique learning opportunities both at home and in the community.

Respite

PCCS’ Direct Support Professionals are provided the knowledge to pay close attention to the needs of each person including behavioral and medical needs if this type of support is required.

Family Education & Training

PCCS’ Family Education & Training sessions provide families with valuable resources and information about service alternatives available to them and how to gain access.

Self-Direction

Self-Direction gives the people and families we support the flexibility to choose the mixture of supports and services that are right for them.

OMH/OASAS

Mental health is very important, and people need a strong circle of support they can always count on to maintain an emotional, psychological, and social well-being.

Consulting Services

PCCS strives to invest in the collective knowledge of your team and help develop into an organizational superstar.

Program Spotlight

Disability Ally Initiative

People often view disability as a negative and personal problem that affects a person. In reality, disability issues and problems come from a person with a disability trying to function in a society that is not accessible. The majority of people living with a disability (and their families) spend a lot of their time advocating for accessibility, the right to receive an education and services they need in school, jobs that they qualify for, the right to be treated fairly by people they interact with, a place to live and build long-lasting relationships, and much more.

When businesses, schools, healthcare providers, and other community-based organizations and members take action to become disability allies through the Disability Ally Initiative training, they give people with disabilities and their families the opportunity to be treated fairly, live in an accessible community, take advantage of job, education and housing opportunities without discrimination, and have meaningful interactions with peers and the rest of the community.

Being a disability ally means providing an accessible community with opportunities in different aspects of life, allowing for a better understanding of people with disabilities and their right to a quality life.

For the past four years, with the help of its generous community, PCCS has crowdfunded and raised enough money to bring smiles to the faces of the students of the Robert Randall School (P.S. 373R). This year, PCCS is faced with one of the most exciting challenges: making sure there is a gift for more than 300 students, 275 school administrators, and 5 school sites! How you can help.
